Short version: Very little. A couple of minutes, once only, for some games.

I dual-boot Windows on my home PC to play games on, and installed it with
the Dvorak layout as the default, so I don't switch layouts at all.  When
it's running, its running Dvorak.  (My fiancee doesn't play games and she
knows how to change layouts in Linux with a little widget by the clock).

For games you'll usually want to use the keyboard keys as the game developer
intended.  For example, pressing the 'R' key on your keyboard to reload
(even though the key now actually means 'P'), and the physical WASD to move
in FPS games.

Some games use the physical keycodes directly, so require no effort to play,
some need you to remap the keys in their option menu before you start.  I've
never run into any game where you can't remap the keys.
