I have a custom plugin for qpsmtpd that I should probably tidy up a bit
and release.

qpsmtpd is wonderful, BTW. It's a perl SMTP proxy which is amazingly
easy to extend by writing small snippets of perl, making experimenting
with spam-stopping ideas trivial. And it's not horribly slow:
http://wiki.qpsmtpd.org/ mentions that apache.org, perl.org, cpan.org
and lists.mysql.org use it.
