I'm sure it can't be the only one.  My qmail setup has no UNIX accounts 
for users either.

- My Setup:
MTA: qmail + procmail (plus fetchmail for auxiliary a/c, and backup MX)
Clamav (clamassassin and clamd) for virus filtering
Spamassassin, and DSpam for spam filtering and per user training
Courier (S)IMAP (plus POP, but it is not used)
Web access via squirrelmail (and Horde, but no one uses it...)

I started on a web based procmail rule filter for users, but it is not 
workable yet.

I'd probably not use qmail again, as I think there are MTAs that are 
more configurable (specifically wrt stopping email during transport).

Courier IMAP was a breeze and seems fast enough.

I cannot recommend DSpam highly enough for per user spam filtering.  It 
is fantastic.  My users just drop email into a couple of folders if 
DSpam gets it wrong/right.  Spamassassin picks up the overtly spammy 
ones first.
